Is there a way to create a three dimensional patterns? Ideally with coordinate systems, for the orientation of the instances?

Yes, this is possible. I would use a table pattern to create this. When creating offet csys you will have access to 3X translations and 3X rotations for each new csys and you can use those dimensions to pattern the csys.
