You have to put the "@[<stuff>@]" around each symbol and text string.
If this is a hybrid of a true GD&T feature with manually added control, you probably can't do that. If it is just added tot he dimension on the drawing, my above method applies.
In other words, there is something wrong with your string. And i did have trouble making the string below. It doesn't seem to like adding the circle-S without adding {0:...} before it. Edit these out if they show up.

You can add all of that info to the feature control frame when you insert Geometric Tolerances.
