Consider which remap file you specified when running afmtopl.
Both the AFM file and the PL file are ascii files you can view
with any text editor. If you have a freshly generated PL file
made from running makeafm and then afmtopl (versus one that was
made by going "backwards" with tftopl), there should be comments
saying what the name of each character is. Hence it should be
easy to see whether these characters are in their expected
locations. To find out what these locations are, see a PL file
for a font that works. (I think, for example, that a left double
quote should be at position 147, but you'd need to verify that.)
