3D Annotate note will not show on screen

3D Annotate note will not show on screen

Trying to create a note flat to screen or any plane for that matter.  My note shows in the combined state detail tree but does not show up on the screen??  Also the note has the suppressed "box" next to it??

3D annotation display is toggled on.

How do I show the note?
