If you have already purchased the upgrade, there are tutorials on upgrading from specific versions. These are typically provided in your account as part of maintenance. They can be access through this page if you have a log-in for the PTC University. If not, see your administrator.

I fyou haven't purchased the upgrade yet, Dale's link is a good place to start. You will also find a ton of videos on YouTube.
