For this idea, I'm proposing something far more limited (so that hopefully it can get added). Add a new object that can be placed inside a symbol. I would call it a "leader attachment point" or similar. A symbol that included this new object would allow a note to be attached to it.
This would allow us to create the Grain Direction symbol with a note outside of the view, as shown in ASME Y14.8.
