Good presentation, though I feel you didn't completely explain why when using holes the revolve feature causes things to be different than extrude feature (don't they both produce "cylindrical" objects???), but anyway, I think the better question would be why does PTC accept that the "best practice" is to have to do more useless work of picking the alternate origin point which has already been picked when the hole was placed on it!
Even worse, the software design often leads to modeling mistakes which are not caught because visually, the error is often not apparent - as your video nicely demonstrates.
At the very least, the system should warn the user that "pattern instances closely overlap - perhaps a different alternate origin needs to be used".
But really, why can't it be that if the feature being patterned is using a point in a "pattern-sketch" as a reference, then that very point should be selected, by default, as the "alternate origin" This has already been requested:
