A colleague just gave me a report with a Mathcad Prime 6.0 document printed as Appendix A. It started on page number 72 and had the page number in the header. I got all excited and asked, "How did you do that?" Simple. They just inserted 71 blank pages at the front of the document and only printed 72 and beyond to the PDF file. Ugh.
