I also find in Creo 4 M030 I still need to regenerate my model 2X for the pushed MathCAD change to showup in the model. Nothing happens on the first regen.

Yes it is really required. Or PTC may add config option auto_regenerate_and_save_mathcad yes/no. So its as per customers choice to make it auto regenrate and save-push yes/no.
