How to create GD&T to be exported to STEP AP242 as GEOMETRIC_TOLERANCE?

How do I add GD&T information to a model in Creo Parametric (Version 5.0)  which can be exported to STEP AP242 to appear as GEOMETRIC_TOLERANCE information like in the attached NIST Sample file?

 

If possible, could please attach a sample prt?

You can add annotation in part using Geometric tolerance under Annotate tab and when exporting to STEP check Annotation to export under step export settings.
