How to track the event of Requirement delete from Requiremen Document?

Hello team,

How to track the event of Requirement delete from Requiremen Document?
In case of many requirements in a ReqDoc some of them could be mistakenly deleted.

How to inform user if this happened?

Is there a way to present a kind of warning screen before requirement delete?
