Does anything show in the MethodServer log, during the timeframe this issue happens? I think that will be valuable for checking.
You should probably open a Support case for this issue.
I imagine, to troubleshoot, Support will probably want some logging turned up to rule out things, during time of issue.

To make sure there isn't a weird permissions/access issue happening here, but perhaps, more relevant:
windchill wt.vc.ShowVersionedData > output.txt
To see what Windchill currently shows for the location/identity info, for the problem object.
Maybe there is a data issue, causing the identity conflict error.
