Single Level BOM PDF No Header Help?

I'm having a problem with running the Single Level BOM report and generating it as a pdf. The header does not continue over to the second page. This only happens with Reference Documents being the only items on the page. Any Ideas? Attached is a dummy file where you can see the problem an page 2.
