Update infotable from Mashup [Thingworx 9.1]

I have a Infotable with Labels: Sr.no, MachineID, Description, Created-by, Time, Status. I want to update the infotable via a Mashup. The Mashup takes input MachineID and Description. There will be a Submit button. After the button is clicked, this information along with current user and time gets added to the infotable. How can I achieve this?

Behind the submit button is a service which adds the row and using the AutoRefresh to redisplay the mashup.
