Further check ThingWorx Navigate Application Log, IR Logs and Windchill's Apache server log, if the request sent from ThingWorx server is successful (200) or failed.
Will the steps differ if Windchill and ThingWorx are not different server with different hostnames ?

In what situation is SSO authentication really necessary for Navigate over Windchill SSL authentication?
(Arshad) SSL Authentication is a lightweight setup. Here Windchill handles the Authentication of each user's login credentials. One drawback here is If Windchill is down ThingWorx cannot be used.
SSO is the most secure configuration. It enables users to be authenticated to Navigate and other system using single login credential.
