blend vertex, manifold from squar to round

I know that I've done this during one of the courde I did
Now I'm unable to reproduce it

I need to create a general straight blend from a rectangular to a
circular section (devided into 4 pieces)

curves of the rectangle need to be "blended" to a single point on the
circular section, the quarter circles need to be blended to the
vertices of the rectangle

I've created blend vertices at each of the 4 vertices of both section
yet I still end up with a twist shape.

I've had errors stating that a blend vertex cannot be at the same
point as the startpoint

Can I do this using the solid general blend or do I need to
build things from surfaces

Can you help

All,

From the replies I received I conclude that I haven't been able to explain
where I'm going.
Attached a top-view picture of something I made with MS-paint.
As you can see two line from each of the 4 vertices on both the circle as
well on the rectangle
In my point of view this means blend vertex
(the triangles are actually flat faces)

Hope this explain better.
