Attribute type for custom input - Change Control

Has anyone defined an attribute type with a yes / no function and if the answer is yes, make the next field active for user input? For my CR process, I want to ask the user "Is there a CAR?" and have the user choose yes or no. If they choose yes, the next text box becomes active and the user can input the CAR number.
