Trying to figure out what is going on here. I have a user who is creating a change notice and almost every time, it creates two change activity tasks that are identical. Are they clicking the button twice to finish or is there some internal bug? Has anyone see this.?

We have seen this behavior and narrowed down the root cause to the end user using an incompatible version of the browser. We are running WC 9.1 and the issue seemed to happen more often when the end user was using Chrome.
