Is anyone using the above ....and if so is it PDMLink "out the box" or modified to your Companies Procedures etc?

I have implemented a few of these at different companies. Chances are you will not be using the system as-is OOTB; you will end up having to make modifications/customizations to your workflow at the very least. Then there will attributes you may want to add.
Some recommendations:
1. Subtype all your change objects and make the base (OOTB) type not instantiable. This leaves you the option to further subtype these change objects for other uses in the future.
2. Keep it simple - don't try to capture every step of your change process in the workflow
3. Don't over automate - some manual steps and user discipline should be expected.

We are using it almost OOTB, only very minor changes.
It is very useful to see red triangle beside objects that have pending changes, so designers will pay attention before reusing it.
Change management in windchill is a very good tool.
