Many replies suggested using "No Duplicates" in the Repeat Region properties and that solved the rpt.qty issue.
For the rpt.index problem, I copied an older table we had and modified the parameters and styles. That doesn't help future users who might search the archives for this problem, but I'm pressed for time and had to push ahead.
