I am not sure I understand what you are asking. Are you trying to make the punch or the part?
When I use a form tool, I make the shape on the fly with surfaces. This works well if you are not trying to maintain a library of punches.

A physical punch tool and the form tool in Creo are two very different things.
So I see several operations that must take place:
1) create the hat parts
2) remove the flange
3) remove the bottom
4) shape the edges (r3.2)
5) round the edges (r0.2)
I do not see what exactly you are having trouble with.

Creo Sheet metal does not make dies. You would model the die using the normal Creo modeling technique. There are no shortcuts for designing physical dies.

As the video suggests, tools like these require some trial and error.
There are people that specialize in making tools such as this.
They do not rely on the CAD models, they rely on how materials behave.
Are you just trying to make these as a standard dental part?
How many do you need to process?
Is each one being custom trimmed and rounded at the side of the gum?
Is this a manual process or power tools?
Are the blanks individual?
Do you need auto-loading onto the die?
etc...
