How do I remove the user image question mark? What does it mean?

Hello, I see a number of my users with a red question mark on their image.

What does this indicate and how do I resolve it?

Do you mean this image: 
 

That indicates that the designated user is no longer a valid user in your system, typically users removed from the underlying authentication realm.

There isn't anything to resolve in those cases, it just lets you know that the user doesn't currently exist.
