I am able to import a word document (test specification document) in to PTC Integrity Test management (test Document and test Cases) but into not able to map contents into Test steps. What am I missing here?

Does the last part of the attachment file name have a pipe symbol ('|') at the start? In other words, is it "|_Toc413928242"? From the screenshot, it looks like that is the case, and that file name is illegal under Windows. I suspect this is why you are seeing this error.
If that character is not a pipe symbol, could you let us know which character it is?

As mentioned by Jeremy i did look into the mapping templates. I do have an error, i am trying to figure out the root cause.
Could you please look into it and let me know if this needs investigation, if it does i shall create a new case. But if its like something minute please guide me i shall try that. The below reported error may be related to this RFC Document - CS166681.

Yes, i too think that it has to do with the name of the attachment file that is blocking the import. I can't confirm if this is a pipe symbol or not, reason being this file is not visible; most of the import related files get created under temporary location. When i begin the import i can see them, if i click OK for the error prompt the folder and files are all gone. Its the same even when the import has completed successfully.
So im really not sure how to resolve this... Do you think creating a case and investigating this further would be the ideal thing to do?
