we are seeing users losing the access to queries and items automatically, even after adding user to the resp groups the access is lost after sometime. we are using Windchill RV&S 12.5.

We have a similar problem: losing permissions temporarily due to failed synchronization with the directory server (LDAP/User/Group Cache refresh).  In our case this seems to be a problem of the Group Cache - users losing their (cached) assignment to a group and herewith also the permissions assigned to that groups.

A successful execution of the "User and Group cache refresh" is repairing this.
