Mathcad Reinstall to new pc

Last week we face issue with our computer Hardware failure. We would like to install and activate Mathcad prime in new pc. Please help us to resolve this issue ASAP.

We tried to “Transferring Mathcad Single User or Student license from an existing Host ID to a new Host ID”

Please find below error..

• Request for a license manually by providing Mathcad product code and physical address of Ethernet adapter using web case logger here: http://support.ptc.com/support/mathcad_supportCenterSingleUser.htm
• The license file will be sent via E-mail.

I facing issue its giving error .

“Thank you for using Mathcad. You have reached the maximum number of license requests available for your product type. To upgrade to the latest version of Mathcad, please visit store.ptc.com”
