New revisions
Resulting objects with dispositions
list of votes with final vote date, role,name of user

Is this an OOTB report? Our Windchill does not have a report like that. Perhaps we simply have a different configuration (this is very likely).
Your report is not a Query Builder report.
The 'Business Reporting' subsystem has changed from Cognos to Jasper.
The OOTB reports in WC 12 are Jasper reports. Any remaining Cognos reports you might still have would be custom reports, and please beware that Cognos support is being phased out.

the Engineering Change Notice report is definitely not OOTB one. especialy with a company logo.
So you should contact a person who is responsible for that report.
System administrator can help you to say who is it.
