I would suggest contacting PTC technical support on this issue to make sure it is addressed properly. One item to keep in mind is that the document must be downloaded via Windchill Desktop Integration, not the browser. Windchill DTI will sync the attributes with the server when the document is opened, the browser will not.
To confirm you are using Windchill DTI, please set the Attachements, File Download Mechanism to Windchill Desktop Integration and not the default value of Basic Browser.
