Can anyone guide me in creating the model as per the below attached file.

What have you got so far? What feature are you having problems with?
I think I would model it as a solid and shell it to have the correct wall thickness

I have tried using revolve option , but unable to create the model.
Possibly an extrude + Blend for lower side of part.
As @Mahesh_Sharma suggests, you will need a blend for the bottom part (square to round).

Meh, not really correct, but the print doesn't appear to be correct either.
