Customer number and sales order number should be in the license file.
If not, you can get them by downloading a customer software report for your site, if you have the rights.
Thanks for the comments. I tried "license management" after my login to PTC, and unfortunately I get this:
Your IT department or system administrator will have to get you set up.
It is a personal license, not on a university system with administrators. I do not have the upgraded access to create the report with the info I need to re-install from the website.
