Mini Toolbar - Annotation not working

There are times, when in Annotation, the mini toolbar will appear, but pressing the buttons fails.  Normally, when hovering over a button, text will pop up giving a description of what the button does.  That does not even happen.  Does anybody else have this issue?

I wonder if I have something buried in my customization's that prevent it from working all of the time.

I have the same problem and didn't find any solution.

Sometimes ESC helps, often not.

It appears over several versions from 19 to 20.

the command for the mini toolbar is

oli:sd-define-application-cmt-contents

Have you already searched your customizations and the lisps you load for a wrong occurrence of the command?
Search e.g. for the textstring

"application-cmt"
With Notepad ++ it is very easy.
