Index for Ebook

I have created an eBook with an index. It is not owrking as expected. When I go to seach, the locations are missing the first letter. nver quite sure if the correct selection was made. Any ideas on how to correct this

I have been following the instruction from the Author's Reference guide. Attached is the:

1. index.txt
   
2. ErrorLog.txt
   
3. ProgressLog.txt
   

The index.txt is after it was sorted using Word.
