rewrite the formula

Hi there.

I am planning to put all the formula in the matrix format. The output I wanted is,  I want to see first the formula before the numerical answer. I am trying to use the symbolic evaluation but I am not getting the result I wanted.  I need some help if this is possible.

When using symbolic eval, did you use "explicit, ALL" ?

All the variable have an equivalent constant value that was  assigned.  The text  I want to see is the formula assigned in the matrix index. Is this possible ?
