PTC Academic Program

After login on PTC Academic Program, I am unable to download Creo student edition.

After clicking on Download PTC Creo, they ask to Fill form.

I am confused plseae solve my issue as early as possible.

    I have not done this in a long time but if i remember correctly you need to fill the form including your host ID in order to obtain a license file.
    But i might be completely mistaken...
