Why are commands grayed out in the shortcut menu but r still available in the quick access toolbar?

Why are commands grayed out in the shortcut menu when they are still available in the quick access toolbar?

this example is how the custom symbol command is grayed out on my shortcut menu but not on my quick access toolbar.

I meant have you changed any configurations related to that menu?

Change to Annotate tab and check if the icon is still grayed out.

Changing tabs does make this option available.

My question is why would it be available in the quick access toolbar regardless of the active tab, but then be grayed out in the RMB menu when not in a specific tab.

Maybe it's a software defect. You should add fixing this as an Idea.
