Documentation for a process release...

I am implementing our company process for Variances and I was wondering if anyone had a proposed standard way of documenting all the pieces involved for later understanding by those not involved with the original implementation.  Looking for something that gathers up the type/attributes added or changed, layout changes, workflows, lifecycles, policies, object initialization rules, etc.  Anyone want to share a template?
