Windchill Error - a datastore uniqueness constraint violation has occurred. -

Would be great if someone could help with this issue. Since I'm using very old version of Windchill, support is no longer providing service. Been searching any article that could help me with the issue but to no avail.

The error is appearing every time a member is added in the "Library" under "Team".
