Forum update: Notify moderator

Another change to go live this Friday July 9th: 

Currently, notify moderator is a feature on the topic menu. By placing a new button in a more obvious place, we want to facilitate and enable more new users to call the attention of moderators. We are aware that this may generate more false positives. Depending on the result, we may change the button a bit and its placement. Overall, this will help the Community to remain clean.
