Extrude placement

How to restore the Placement pop up window for the Extrude function. It was accidentally moved and now cannot be accessed when clicking on the Placement tab.

    Reset Panel Positions using: 

     

    File > Options > Windows settings > Click Reset Panel's Position 

     

    Or clear UI file in cache folder
