Creating a Punch in Sheetmetal

Hi, I'm trying to create a Punch in my sheetmetal but I kept getting a message "could not construct feature geometry" maybe b/c i kept messing with it. I followed everything like my instruction video said but kept getting same message. Some other messages also appeared: "WARNING: Some constraint relations are no longer satisfied in SHEETMETAL." "Can not make a Form..." Appreaciate for the help, thanks
