Which is the proper element to profile for the Index?

When profiling in the prolog for the Index, which element should be profiled: metadata, Keywords, or indexterm? When publishing to pdf, the profiling on the metadata seems to be ignored. Is that normal?

Profiling seemed to work at the metadata element.
