License manager can't initialize. No server lines in license file.

Where does the server/daemon information get added on a Mathcad/PTC license file ?

I'm getting an error 1067, and my log says:  License manager can't initialize. No server lines in license file.

Hi,

if your license file does not contain SERVER line then your license is locked. In this case FLEXnet is not installed. Mathcad installation is informed that license is located in the license file.

Martin ..... where do the server & daemon lines belong in the license file. 
Can you show me a sample, or a sample license file.
