Reached the maximum number of license request

It says that I've reached the maximum number of license request available from my product type. Please help me

    Hello,

     

    You should request for a license manually by providing your Mathcad product code and physical address of Ethernet adapter using the following web case logger: 

     

    http://support.ptc.com/support/mathcad_supportCenterSingleUser.htm

     

    The license file will be sent via E-mail.

    After clicking the case log, selecting the version, entering the product ID, and pressing Enter (not "NEXT"), a case logging window appears. Yay, hopefully I can get an email with a new license.
