Thomas Algorithm : This array index is invalid for this array

Hello,

I was learning the Thomas algorithm and I got stuck at a point where it says "This array index is invalid for this this array"; I check the system but I couldn't find such a problem causing anywhere. I attached the mathcad code; any assistance will be appreciated
