Re-installing Mathcad 15

Need to re-install Mathcad 15 after a reset of my machine, and I already have a license. But I do not have a customer number nor sale number .

Customer number and sales order number should be in the license file.

If not, you can get them by downloading a customer software report for your site, if you have the rights.

Thanks for the comments. I tried "license management" after my login to PTC, and unfortunately I get this:
