Mini Toolbar - Annotation not working

There are times, when in Annotation, the mini toolbar will appear, but pressing the buttons fails.  Normally, when hovering over a button, text will pop up giving a description of what the button does.  That does not even happen.  Does anybody else have this issue?

I wonder if I have something buried in my customization's that prevent it from working all of the time.
