there is simple solution ... please follow below mentioned steps:

0.] remove all files from hidden directory C:\ProgramData\PTC\Licensing

1.] download Creo 12.0 installation package from https://support.ptc.com/appserver/auth/it/esd/academicFree.jsp page

2.] install Creo 12.0

use BE120908EDSTUDENTEDK12 code

use BK120908EDUNIVERSITYED code

https://apps.ptc.com/schools/references/install_creo_university_edition.pdf

3.] backup license file generated by step 2.

file is located in hidden directory C:\ProgramData\PTC\Licensing

4.] uninstall Creo 12.0

5.] install Creo 5.0

use license file generated by step 2.
