How do you export images from PTC into an Excel spreadsheet? I've read that this is not an out of the box function. I am looking for methods

By PTC, I assume you mean PTC Integrity.

This is not currenlty possible, either through the UI Export to Excel functionality, or via the Excel Integration. Images would have to be part of a rich text field, and Excel support of Rich Text content is somewhat limited, but RFC 147971 covers both importing and exporting rich text elements. It does not explicitly cover images.

To add your organization to RFC 147971 or to create a new feature request to cover images in particular, please contact PTC Integrity Lifecycle Manager Support to open a case to do so. Please let the TSE who picks up the ticket know about this thread in the Community.
