Cannot create new user in Windchill

I am using Windchill PDMLink Release 11.1 and Datecode with CPS M020-CPS04 I am trying to create a new user in Windchill 11.1. When i go to site - utilities - participant administration - create user is does not give the fields to enter name - first, last name, etc. It starts with postal address and no option to enter a username.
