Custom Change Request Information Screen

The prior admin created a custom entry screen for Change Request information but left no documentation as to where it was located or what he actually did. I tried searching for some text in the form, but found nothing. Does someone know where a modofoed form may be stored in Windchill? This was done at V9 and been migrated to 9.1, then rehosted to 10.0. The upgrade manager found the file because it did make it to the V10 server.

Has anyone modified the screen for New-Change Request or do you leave them in the OOTB state and modify your process to the form and workkflow?

We actually had a java app along with a custom jsp page written for the information that we need. It also does an end item where used collection of only active items. Actually, one shouldn't use the OOTB workflow. You should always at least copy it to your own workflow. This is so that if PTC changes something in the OOTB, which they have, it doesn't affect you. Our workflow though is custom to our processes. We did have to do some give and take though between keeping old processes and improving to new processes. Some things just were not relevant anymore.
