We changed the lifecycle of objects through New Promotional Requests.
Initially, the new promotional request went smoothly.
If you made the second upgrade request, access was denied and access denied.
Requests for new promotions with the product manager will work smoothly.
Why is not a member requesting a second promotion?

It seems like user does not have modify or set state permission for second promotion request. You can try changing below preference

Also make sure you have necessary promotion transition defined in lifecycle

Was the first promotion request approved? Typcially it should be approved first then you can create another one if you need.
